*VANCOMYCIN DOCUMENTATION CRITERIA

Use of Vancomycin for surgical prophylaxis requires MD, NP or PA documentation of one or more of the following reasons before surgery start. 
These reasons are listed on the Surgical Required Elements (Green Sheet). By checking one or more of these boxes on the green sheet before surgery start, the pre-operative documentation requirement is met.

 
  • Beta-lactam/PCN allergy
  • Increased MRSA rate facility-wide or operation-specific
  • MRSA infection or colonization or positive screen or history
  • Valve surgery on O.R. consent
  • Chronic wound care or dialysis

ORAL antibiotics are only acceptable for Prostate Biopsy & Pubovaginal Sling
